If you are currently our patient and taking Adderall®, Ritalin®, or Concerta® (or another controlled substance on this list), make sure to have a qualified referral on file before November 7, 2024.

You can either provide this form to your local clinician and ask them to complete and submit it, or you can ask them to send a referral directly to us for you to continue your treatment with ADHD Online.
