If you are a current patients receiving care via telehealth and prescribed a controlled substance you will need a qualifying telemedicine referral by a clinician, such as an MD or DO, or an Advanced Practice Nurse or a Physician Assistant, who has performed an in-person examination.
Your primary care doctor can provide a direct referral to us, as well.
You must have the form on file with us by November 7, 2024.
