How to Submit an Accommodation Request:
From Our Website:
- Select "Patients" from the menu bar.
- Click "Submit a Request" from the dropdown.
- Choose "Documentation Request".
From your Patient Portal:
- Go to the Support section.
- Click the “Documentation Request” link under the “For Fastest Response” section.
Helpful Tips:
- Complete all required fields in the form.
- Attach any forms provided by your school or employer, ensuring the patient information sections are filled out before uploading.
- Include all details required by the requesting institution.
- Once your request is submitted, it will be routed to a team specialist and your clinician for processing. Please allow up to 14 business days for completion, and submit your request well in advance of any due dates.
Important Notes:
- Clinicians can only support accommodation requests for the conditions they are treating. These requests are reviewed and completed at the clinician's discretion on a case-by-case basis.
- If your form requires a face-to-face visit, you will need to schedule and pay for a follow-up virtual visit with your clinician to complete the request.
