We understand that living with ADHD can impact your life in many different ways. We are happy to request accommodations on your behalf. We recommend that patients reach out to their work or school first to see if they have a specific form that needs to be completed or any other requirements.
To submit a request, please:
- Log into your ADHD patient portal.
- Click on the “Support” link in the left navigation bar.
- Select the “Documentation Request” link under the “For Fastest Response” section on the support page.
- Complete the required fields. Please ensure that you attach any forms that need to be completed and include any other details that are required by the school or employer. Patient information sections must be completed before uploading a school or employer-provided form.
Once you complete the request form, your request is routed to our team specialist and your clinician for processing. Accommodation documentation can take up to 14 business days to complete, so be sure to submit your request ahead of the due date accordingly.
Our clinicians can only support the accommodations request for the conditions that we are treating. All accommodation requests are completed at the sole discretion of the clinician on a case-by-case basis.
If your form requires a face-to-face visit you will need to schedule and pay for a follow-up virtual visit with your clinician in order to complete the form.
