What is the AVS?
The After Visit Summary (AVS) is a document summarizing key details from your recent appointment. It provides an overview of your care plan and serves as a reference for follow-up steps.
When and where can I find my AVS?
Once your provider finalizes your appointment notes, you will receive an email notification letting you know the AVS is available. To access it:
- Log in to your patient portal account.
- Navigate to the "Appointments" section.
- Locate and click on your most recent visit under "Past Appointments" to view or download the summary.
What information does the AVS include?
The AVS contains specific fields from your chart notes, including:
- Plan/Next Steps: Your provider's recommendations and instructions for follow-up actions.
- Medication Changes: Updates or modifications to prescriptions, if any.
- Referrals or Follow-ups: Details on additional care, referrals, or when your next appointment is due.
- Educational Resources: Links or summaries of educational content related to your care.
How can I use the AVS?
